Making New Patient Appointments

As part of our commitment to providing convenient, high-quality care, we are transitioning to telemedicine appointments for all new patients. Telemedicine allows you to meet with your healthcare provider securely using your phone, tablet, or computer from the comfort of your home.

Telemedicine visits are well-suited for many types of appointments, including new patient consultations, follow-up visits, medication management, medical record review/second opinion for complex allergy conditions, ordering allergy blood testing, and discussing test results. If an in-person visit is medically necessary, we will always let you know and help to arrange for it with other providers. Our team will provide clear instructions on how to access your telemedicine appointment and is happy to assist you with any technical questions ahead of time. Thank you for choosing our practice. To make your first appointment, please complete the brief new patient interest forms.  Click the link under online forms for New Patient Forms to access the online forms. Fill out all required information as completely as possible. When finished, click Send — your information will be sent securely to our office.  Please allow 1-2 business days to review your forms before receiving a call back from our patient coordinator.
